Below are all relevant CAS handouts you will need for your CAS programme even though you will complete your reflections on your managebac.com account.
Remember, you MUST submit a supervisor agreement form to your advisor at the beginning of each experience and then provide the supervisor's evaluation of you at the completion of each project or experience.

Advisor and Supervisor Paperwork
For each CAS experience proposed during an interview, a supervisor agreement form must be completed and submitted to your advisor for the activity/project approval, including any experience/project completed in UAIS. In the case of a solitary activity you plan to have your advisor supervise, the completion of this form is not necessary; however, you must propose a documentation system that you and your advisor agree is appropriate to document the completion of the experience. Remember to be clear to any outside supervisor that you need their commitment to submit a supervisor completion form once you are done with the CAS experience.

The form below serves as a hard copy of the online supervisor completion form. This is NOT to be used UNLESS a supervisor wishes to sign off on your experience or project in person rather than online via ManageBac. If this situation arise, print the form below, give it to your supervisor to complete by hand, and then take the completed form below to your advisor during your next interview, who will make a note of it in managebac and complete the experience for you.
